Mandatory requirements
- Copies of VAT and Tax clearance certificates
- Copy of certificate of incorporation/registration
- Postal, physical, email address, telephone and fax numbers
- A minimum of three trade references
- Full Banking details, audited financials & PACRA annual returns both for the last 2 years.
- Names of Directors and their residential physical addresses
- Registration with relevant statutory and regulatory bodies where required

Conditions
MTN shall undertake a preliminary evaluation and only successful firms will be notified. Applicants must clearly state areas of specialisation on the envelopes and MTNZ restricts the limit to a maximum of three categories per company. All applications must be labelled 'Registration of Suppliers and Contractors' and be deposited in the tender Box stationed at Maanu Centre third floor or mailed to P.O Box 35464 Lusaka.
Interested suppliers, contractors and consultants are requested to download the application form from the link below and at the point of submission, pay a non refundable fee of K200,000.00 from the Procurement & Logistics Department, third floor, Maanu Centre, Stand No. 4647, Beit Rd – Addis Ababa Roundabout, Lusaka – Zambia and MUST enclose copy of the receipt before depositing in the tender box. Faxed submissions and those without a receipt will not be accepted. Submission of application forms and payment can be made at the address below:
